I've been meaning to post this up for years ever since I left the church; this seems a good a place as any. I did my best to recreate the original as close as possible, typos and all.

Patriarchal Blessing
Patriarch: W. Craig Jones - 2006
Stake: Cedar City Utah West
Brother **************, by authority of the Melchizedek Priesthood and as an ordained Patriarch in the church, it is with pleasure that I lay my hands upon your head to give you your patriarchal blessing; this blessing will serve you as a spiritual compass to give you direction in this and the life to come; it will comfort you in times of discouragement, in times of decision and in times of uncertainty. By your faithfulness it serve to let you know that the Lord lives and the Lord knows you. He knows you by name. He knows you thoughts. He knows your heart. He knows your needs and He knows the trials and temptations and difficulties that will part of your life, as it is a part of all lives, for that is part of His plan here in mortality.
You are of the lineage of Ephraim within the House of Israel; this is a great blessing given to you as a result of your pre-mortal life; there you progressed by being righteous, by making choices that led to your growth and spiritual progress. You learned to use you agency wisely, and you made that great choice to follow the plan of our Father in Heaven and His Son Jesus Christ. This plan included coming to live here in mortality, to be tried and tested and to determine if you would so the things that our Father in Heaven would desire you to do.
You have also been reserved to come forth in the latter-days; you have been reserved because of your strength, your talents and your gifts, your courage to do right and to follow the Plan of out Father in Heaven. You have been blessed to be born of good descendents here in mortality, in which there is a long line of faithful, dedicated church members; you were blessed to be born of goodly parents into family that cares about you, a family who loves you, who have taught you and who have set a good example for you; and who are concerned about your future welfare. You have been a good and valiant son and grandson; you have been respectful, courteous and thoughtful of your parents and ancestors.
Now as part of you lineage, you have a mission to accomplish in this life: to be a valiant member if the church, to develop a strong testimony if the truth of the church; to gain a conviction of the truth of our living Prophet and prophets that have gone on before; to be a father and good parent to children.
By your worthiness you will be guided to choose a companion that will be a strength to you; she will be one who will live with you in unity; take this companion to the temple of the Lord; make that a goal in your life, so that your marriage will be a covenant marriage and an eternal marriage. Live faithful to the covenants that you make there for you not only will covenant with your companion but with our Father in Heaven. Part of this covenant is to treat each other as He would have you, to love each other, to be loyal and faithful to each other. By living the Gospel and staying active in the church, you marriage will be a strong marriage and an enduring marriage.
You will be blessed with children and these children, if you teach them and set a good example for them, will be your greatest source of joy and your greatest treasure in this life and the life to come. The Lord will bless you to be a good father, a father who is kind and patient and will take the time to teach your children.
You will also be a leader in the Church. The Lord has given you gifts of leadership, of courage , of confidence, of discernment of people's needs, of faith and of trust. Live worthily of these great gifts, for they are given to you for your spiritual progress and for your service to others. If you will accept callings in the church, you will learn to fully love the Lord, by serving Him as you serve others. As you develop these gifts in your life the Lord will add to your gifts. He will do so through His spirit so that you might progress throughout this life and the life to come.
Now as you face the important decisions that lay ahead of you, the Lord will bless you with His spirit. You have been given the Gift of the Holy Ghost at the time of your baptism; live worthy of this great gift, be aware that the world does not understand the Gift of the Holy Ghost, but you will be blessed to feel that influence, that comfort, that guidance in your life. It is through the Gift of the Holy Ghost that your testimony will become firm, that you will know that our Savior lives and guides this Church and that your Father in Heaven is real. Remember that you are a spiritual offspring of heavenly parents and they are to be your example. He has sent His Son to provide the path, the direction and the example for you to follow.
Among your mission also will be to develop Christ-like character traits. Study the Savior's life, learn of His insights into how to treat people and how to bring people happiness and joy. The Lord has also given you other examples to follow such as the Prophet Joseph Smith. Study his life, know of the truth of his mission; know that he saw and bore record of the Father and the Son and brought forth the greatest recorded witness of Christ in the Book of Mormon; by regular study of this great book, you will gain an unwavering testimony of the divinity if Joseph Smith's calling as a prophet.
Now I bless you that you will live worthily, that you will keep yourself morally clean, that you will take time to study the scriptures, that you will pray regularly; these are the ways that you will stay close to the Spirit and retain a strong testimony. And by doing these things you will be blessed to come forth on the first day of resurrection, which is the resurrection for those who are Just and Righteous. This will qualify you to live in the Celestial world. I seal these blessings upon you with great love and appreciation for the privilege in the name of Jesus Christ Amen.
