Brother ________, in the authority of the Holy Melchizedek Priesthood which I hold as a patriarch in the _____ stake, I lay my hands upon your head and give unto you your patriarchal blessing.
_____ , the Lord is pleased that you have sought this blessing from Him at this time, and He is desirous of giving you all that you need to fulfil your purpose here upon this earth, for you are a son of eternal parentage. You did live in the eternities before in a pre-earth life with your heavenly parents, and there you were nurtured and cared for as you grew to that stage of development where it was essential that you come to this earth to obtain a mortal tabernacle and to prove yourself worthy of eternal life.
You were there at the council of heaven. You heard the plan of salvation revealed to you, and you shouted for joy. You were among those whom the Lord chose to send to this earth to help carry out the plan of salvation that all who will may obtain eternal life. You fought valiantly against Lucifer as he rebelled and was cast out of heaven. you were a faithful son who followed closely the counsel and direction given you by your heavenly parents, and it was decreed that you should come to this earth at this period of time, born through the lineage of Ephraim, that you may be an instrument in the hands of the Lord in preparing for his return to the earth.
He is grateful that you have accepted the responsibilities of the Aaronic Priesthood, and as you carry out your duties and responsibilities in this priesthood, you will become qualified to receive the Melchizedek Priesthood, the power to act in the higher ordinances of the church, the have the authority to go forth to proclaim the message of the restoration to the world.
You have been born of goodly parents who have given you the opportunity to learn and to gain a knowledge and understanding of the laws of this world, and of the plan of salvation. Follow their counsel and teaching, and as you honor your earthly parents, so will your Heavenly Father honor you.
Seek an education in all fields that are available to you. Make the Book of Mormon a part of your daily living, and your study. The word of the Lord is the greatest knowledge that you can obtain here upon this earth, and it will guide you in your chosen field, that you may become qualified to work, that you may provide the necessities and comforts of life for those who will depend upon you.
Live the Word of Wisdom, that your body may be strong and healthy. That your mind may be alert and active. Do not follow the ways of the world. When means are made available to you, pay an honest tithe, and be generous in your offerings, and the windows of heaven will be opened in your behalf, that you may never want for the necessities and comforts of life, that knowledge will be poured down upon your head, that you may be able to expound the gospel truths to the world.
I bless you with the gift of tongues that you may speak clearly and understand those whom you will teach and bless by the power of the priesthood which you will hold.
I bless you with the power of discernment that you may separate truth from error. As you continue your education you will be exposed to many false doctrines and philosophies. Listen the the promptings of the Holy Ghost, and you will be guided and directed that you may avoid the pitfalls of life, that you will be protected from harm and accident, that you may fulfil your mission here upon this earth. Live worthy to go to the House of the Lord and receive additional blessings. Pray for guidance to find your eternal companion to assist you in the furtherance of the Lords work here upon this earth, that you may be sealed for time and all eternity in the House of the Lord. You will be a light and a guide to many.
Prepare yourself. Accept every opportunity to work in the church, and in the civic affairs, that you may be able to help others. Service is the key word to success.

I was 3 months away from turning 14 at the time I received this blessing.
At the time I was really really worried about giving a talk in church
and was happy to see that I would be able to speak clearly to be understood and also to understand. I served a mission in Argentina after failing high-school Spanish. to this day my vocabulary in Spanish is mostly Christ-centric.
I took paragraph 6 very serious. My dad was the same way always wanting to know everything. He taught me to ask too many questions and everything I learn or explore always get's filtered through the Book of Mormon and other scriptures.
The third from last paragraph is still funny to me. With out much fore thought I changed majors from computer science to Outdoor Education and Youth Leadership. Which landed me a job working with the Boy Scouts. I have never been asked to work in civic affairs and that's just fine with me. But I did work as a night janitor for the church while going to college.
